Stakeholder Partners

PCIC seeks collaborative partners to join us in finding innovative solutions for improving healthcare quality and costs for the most vulnerable in our community. It’s through these partnerships that we've built the initial data and communication platforms necessary to gain new insights and develop interventions that impact people's lives. There are several ways to partner with PCIC, they are:


Types of Partners

Hospitals

Through hospital partnerships PCIC is identifying top High-Need, High-Cost patients of individual and multiple hospital systems and coordinate care through our intervention program to reduce utilization and costs across the system.

Managed Care Organizations

Through MCO partnerships, PCIC is identifying and utilizing claims data towards identifying High-Need, High-Cost patients across systems and coordinating care through our intervention program to reduce utilization and costs across the system.

Social Service Agencies

Through Social Service agency partnerships, PCIC is identifying and integrating the social determinants of health for top High-Need, High-Cost patients with clinical data, and beginning to coordinate interventions.

Police & Fire

Through Police and Fire partnerships, PCIC is identifying top High-Need, High-Cost patients throughout the community based on EMS call and transport data which can be overlapped with multiple partner data sets and built into powerful dashboard insights.

Current Projects

Community Health Choice
In partnership with Community Health Choice, PCIC has jointly identified a group of their High-Need, High-Cost patients through data analytic dashboards, and are currently managing the clients through our care coordination program.
In partnership with the Houston Recovery Center, PCIC is building powerful dashboards and analytic insights with multiple agencies such as hospitals, clinics, Police and Fire, and other social service agencies.

Houston Methodist Hospital
In partnership with Houston Methodist Hospital, PCIC has jointly identified a group of their High-Need, High-Cost patients through data analytic dashboards, and are currently managing the clients through our care coordination program.
In partnership with the Harris Health System, PCIC is working with their High-Need, High-Cost patient program to stratify clients based on utilization, costs and diagnosis to build appropriate interventions based on the needs of each tier. Clients are identified and stratified through PCIC's powerful dashboard analytic insights.

Secure a Data Sharing Agreement (MOU/BAA Business Associate Agreement)

Build Data Analytics & Dashboards
  • Provide insights across systems through customized overlap analysis
  • Identify High-Need, High-Cost patients
  • Develop dashboards at the patient, provider, institution, and community levels

Step 1

Develop a Care Coordination Program for High-Need, High-Cost patients through a CSA (Collaborative Services Agreement)
  • Eligibility, Enrollment & Recruitment
  • Intervention management
  • Assessments and outcomes reporting
  • Graduation and warm "handoff" back to partner

Choose a Payment Model
