Values-Based training - 1/9/19 and 1/10/19

PCIC hosted a two-day workshop focused on our “Values-Based Care Model”. Participants were provided with cognitive-behavioral and complex trauma frameworks to gain a greater understanding of why complex patients behave in ways that seem self-destructive.

With our expert colleagues at the University of Southampton and University of Houston and our own care management intervention team, we have developed a Values-Based Care Planning health record system that elicits patient goals in terms of overall health, as well as the many barriers that stand in the way of patients achieving their goals.
