One Month In: How ICJ Self-Service Is Expanding Access for Houston’s Working Families

One month ago, United Way of Greater Houston and Patient Care Intervention Center (PCIC) launched the Integrated Client Journey (ICJ) Self-Service portal—a new way for Houstonians to set goals, find help, and work toward financial stability at their own pace.

The launch is more than a tech milestone. It’s a response to a very specific problem in Greater Houston: thousands of households are working hard and still can’t consistently cover the basics.

Connecting Communities with Resources: Updates on PCIC’s Partnership with Houston Food Bank

At Patient Care Intervention Center (PCIC), we know that health and food security are deeply connected—and addressing both requires cross-sector collaboration. That’s why our partnership with Houston Food Bank continues to grow through two flagship programs: Houston Food Bank’s Community Assistance Program (CAP), now live and expanding, and their upcoming Medically Tailored Meals (MTM) program, launching later this year.

Together, these initiatives are creating scalable, data-driven solutions that improve access to nutritious food, strengthen community health outcomes, and reduce costs for healthcare systems.

Effective communication is the backbone of successful care coordination. To help programs stay connected with both staff and clients, PCIC is excited to introduce the Announcements Module—a powerful new feature designed to streamline communication within the Unified Care Continuum Platform (UCCP).

With this module, authorized users can send important updates via email, SMS, or voice messages to internal teams and program participants. Whether it’s a critical service update, a reminder for upcoming appointments, or a general announcement, this feature ensures that the right message reaches the right audience at the right time.
